Consumer Driven Health Care
Defined narrowly, consumer driven health care refers to health insurance plans that allow members to use personal Health Savings Accounts, Health Reimbursement Arrangements, or similar medical payment products to pay routine health care expenses directly, while a high-deductible health insurance policy protects them from catastrophic medical expenses. High-deductible policies cost less, but the user pays routine medical claims using a pre-funded spending account, often with a special debit card provided by a bank or insurance plan. If the balance on this account runs out, the user then pays claims just like under a regular deductible. Users keep any unused balance or "rollover" at the end of the year to increase future balances, or to invest for future expenses. This system of health care is consumer driven health care because of routine claims using a consumer-controlled account versus a fixed health insurance benefit. That gives patients greater control over their own health budgets. In the consumer-driven model, consumers occupy the primary decision-making role regarding the health care they receive. Consumer driven health care received a boost in the U.S. in 2003, with passage of federal legislation providing tax incentives to those who choose such plans. Proponents argue that most Americans will pay less for health care in the long haul under consumer drive not only because their monthly premiums will be lower, but also because it increases free-market variables in the health care system, fostering competition, which in turn lowers prices and stimulates improvements in service.The Medicare Prescription Drug Improvement and Modernization Act includes provisions to stimulate the popularity of these plans. The law expanded medical savings accounts, renaming them Health Savings Accounts and created tax incentives to encourage adoption of high-deductible health plans. Banks were empowered to create accounts, which deliver tax-free interest to the holders, who can then withdraw money tax free to pay for qualified health care expenditures. To qualify for an HSA, the purchaser must also have a qualifying high-deductible health insurance plan. Participants contribute more to the savings account than would be required to fulfill their annual deductible, and any unused portions of the account accrue without tax penalty so long as the funds are only for qualified medical expenses. Urticaria
Urticaria, or hives, is a skin condition commonly caused by an allergic reaction, characterized by raised red skin wheals (welts). Hives are a skin-disease--itis or --uredo. Wheals from urticaria can appear anywhere on the body, including the face, lips, tongue, throat and ears. The wheals may vary in size from about 5 mm (0.2 inches) in diameter to the size of a dinner plate; they typically itch severely, sting or burn, and often have a pale border. Direct contact with an allergic substance or an immune response to food or some other allergen, causes urticaria, but it also appears for other reasons, notably emotional stress or innocent events, such as mere rubbing or exposure to cold.

Rape Fantasy
A rape fantasy or ravishment is a sexual fantasy involving imagining or pretending being coerced or coercing another into sexual activity. In sexual role-play it involves acting out roles of coercive sex. Rape pornography is literature or images associated with rape as a means of sexual arousal. 45.8% of men in a 1980 study reported fantasizing during heterosexual intercourse about a scene where [they had] the impression of being raped by a woman 3.2% often and 42.6% sometimes, 44.7% of scenes where a seduced woman pretends resisting and 33% of raping a woman. Where male rape fantasies centre around raping rather than being raped, they may bring sexual arousal either from imagining a scene in which first a woman objects but then comes to like and eventually participate in the intercourse, or else one in which the woman does not like it and arousal is associated with the idea of hurting the woman. A study of college-age women found over half had engaged in fantasies of rape or coercion which, another study claims, are within the normal range of female sexuality. One form of sexual role-playing is the rape fantasy, also called ravishment or forced sex role-play. Ravishment has become a more preferred term in BDSM circles, as it makes a distinction between consensual role-play and non-consensual assault. Though consensuality is an important component of sexual role-play, the illusion of non-consensuality i.e., rape is important to maintaining the fantasy. Crossing the line may constitute an assault. Since the illusion of non-consensuality is important to the fantasy, one or more safe words are typically employed. This way, a participant can protest without stopping the scene, unless the safe word is used. Often a variation on the stop-light system is used, with different colors designating different messages: red to stop everything, yellow to slow down or take it easy, and so forth. For scenes where there is an element of surprise, the top or ravisher may use a start word or other identifying signal. In consensual ravishment scenes, all participants carefully negotiate what will transpire beforehand. Limits are respected and made very clear, to maintain safe, sane and consensual play. Such negotiation would also include discussion of emotional issues for both partners, especially if there has been a prior history of actual sexual abuse or assault.
